If you are a participant under the AFPI medical, dental, or vision plan, your benefits will terminate on your final day of employment.  If you are enrolled in any of these benefits, you are eligible to continue this coverage through COBRA.  COBRA is administered through the vendor, Wex. You will receive information about COBRA enrollment and rates via the U.S. Postal Service to your address on file with ADP within 2 weeks of your termination date.  COBRA election and payments are made directly to Wex.


Benefit coverage ends of your last day of employment with AFPI.  After your coverage ends you will have 60 days from that date to elect COBRA.  Regardless of when during those 60 days you elect, the coverage will be effective retroactive to the date after your active coverage ended so there is no gap.  You will be eligible to continue COBRA for up to 18 months (as long as you pay the premiums in a timely manner).
